pH with Impedance Study - The Gold Standard for Reflux Testing
The 24-hour pH with impedance study is an advanced test that measures both acid and non-acid reflux in the esophagus. Unlike the standard pH test, which detects only acid, the impedance part can identify even weakly acidic or non-acid reflux, including gas or a mix of liquid and gas. This makes it especially useful for patients who continue to have symptoms despite taking acid-suppressing medicines or those with non-erosive reflux disease (NERD). It also helps track how far the reflux travels and how it relates to the patient’s symptoms.
The test, combined with symptom recording, allows doctors to better distinguish NERD (where reflux causes symptoms) from functional heartburn (where symptoms are present without significant reflux). It is also the only test to confirm that a patient is having LPR (Laryngo-pharyngeal reflux)
Overall, pH with impedance gives a more complete and accurate picture of all types of reflux episodes than a pH test alone.
What Should Patients Know Before 24-Hr pH Impedance Testing?
A 24-hour pH impedance test is a specialised diagnostic procedure that requires some simple preparation to ensure accurate results. Proper pre-test instructions help in obtaining reliable readings of both acid and non-acid reflux episodes.
Patients should schedule an appointment and receive appropriate pre-test instructions. Regular medications being taken should be discussed with the doctor. The doctor may advise stopping certain PPI medications 7 days before the test. If stopping these medications causes significant discomfort, medical advice should be sought, and alternative medications may be prescribed temporarily.
On the day of the test, patients are advised to stop eating and drinking at least 4 hours before the procedure.
